It is possible to change your mind at any time by speaking to your GP practice or by printing and completing the form using the link below and returning it to your GP practice.
You can object to sharing your information electronically using JUYI; however this may result in delayed or sub-optimal care, particularly if you are incapacitated.

If you were registered with a Gloucestershire GP before March 2016 you will have received a letter asking if you want to share your information using JUYI, and your preferences will have been recorded.
If you registered with your GP practice after March 2016, you will have been asked for your preference on sharing information using JUYI.
